Toni L. Eaton, serves as the President & CEO of Old Colony Hospice in Palliative Care. Toni brings extensive expertise to the organization through her many years of clinical leadership experience. She oversees all operational aspects of the Hospice & Palliative Care Programs at the organization. Joining the organization in 2006, Toni served as the Vice President of Business Development where she proactively identified, established and nurtured positive and responsive working relationships with new and established referral sources. Prior to working at Old Colony Hospice, Toni served in the United States Army Reserve Nurse Corps as First Lieutenant. As a Registered Nurse at the Carolina’s Medical Center in Charlotte, NC., she worked on the Organ Recovery/ Transplant Team as an Organ Procurement Coordinator. In 1998, she became a Specialty Case Manager with Tufts Health Plan and was promoted to Project Manager. Toni received her Diploma in Nursing from Brockton Hospital School of Nursing, Bachelor of Science Degree in Nursing from Queen’s College in Charlotte, NC and her Master of Science in Leadership from Regis College in Weston, MA. Toni also holds a Certificate of Professional Fundraising from Boston University. Toni has received many honors including receiving the South Shore Women’s Business Network Award and being featured on Magic 106.7’s Exceptional Women Program. She currently sits on the Hospice & Palliative Care Federation of Massachusetts Board of Directors as well as the Brockton Hospital School of Nursing Alumni Association Board of Directors.
Kathrine Kelly joined Old Colony Hospice & Palliative Care in 2010, and has over 20 years of experience in business administration. As a key leader within the organization, she collaborates closely with the President & CEO to develop and implement operational strategies that drive the achievement of organizational goals.
In her current role, Kathrine oversees multiple critical departments, ensuring efficiency, compliance, and high-quality organizational operations. She supervises and provides oversight to the Palliative Care Department, the Volunteer Department, the Medical Records Department and the Information Technology Department.
Kathrine holds a Bachelor of Science in Business Administration from Bridgewater State University and earned her MBA from Southern New Hampshire University.
Samantha Brooker, LICSW-ACHP-SW, Vice President of Patient Access and Support Services, joined Old Colony Hospice & Palliative Care in 2015. Samantha has more than 12 years of experience in Social Services. She leads a compassionate team of Social Workers, Spiritual & Bereavement Counselors, and a Volunteer Coordinator. Working closely with the VP of Clinical Care, Samantha provides guidance, support, and in-service education related to the psychosocial aspects of healthcare. Samantha earned her Bachelor of Social Work and Master of Social Work from Wheelock College.
John Zalis, Director of Human Resources, joined Old Colony Hospice in 2020. John has more than 23 years of experience in Human Resources. He is responsible for directing all Human Resource functions, including recruitment, employee relations, performance management, staffing and benefits. He works closely with the President & CEO and the other members of the leadership team to ensure appropriate polices are in place and that the organization remains compliant with all Human Resource related matters. John graduated from Salem College and holds a certificate in Human Resources Management.
Heather L. MacDonald began her journey with Old Colony Hospice in 2004 as a per diem RN. Recognizing her true calling in hospice care, she transitioned to a full-time RN Case Manager within six months. In 2015, her dedication and leadership were honored with the prestigious Spirit of Hospice award.
Her leadership path began in 2009 when she stepped into the role of Patient Care Coordinator, ultimately leading to her current position as Director of Nursing. With over 20 years of hospice experience, she is passionate about mentoring and guiding the next generation of hospice nurses.
Heather is a graduate of Massasoit Community College of Nursing and earned her Bachelor of Science in Nursing from UMASS Dartmouth in 2019.
Katie Minoie joined Old Colony Hospice in 2015, starting as a dedicated weekend nurse. Her passion for compassionate patient care and her commitment to excellence quickly led her to take on the role of RN Case Manager. Katie now holds the role of Director of Admissions, overseeing the intake & admission departments and other organizational departments.
Katie holds a Bachelor of Science in Nursing (BSN) from Southern New Hampshire University and a Master of Science in Nursing (MSN) from Western Governors University. She is also a Certified Hospice and Palliative Nurse (CHPN), showcasing her expertise and dedication to providing exceptional care to patients and their families during life’s most vulnerable moments.
Katie's extensive knowledge and compassionate approach make her an invaluable Old Colony Hospice team member, ensuring that every patient receives the highest standard of care.
Started as an ICU nurse in New York in 1982. She moved to FL to continue her career in the ICU. Susan moved into the home health industry in 1993 and then into the hospice sector in 1997 as a compliance vice president. She moved to Massachusetts in 2005 where she continued her career in the clinical hospice sector where she expanded her role as the Chief Compliance Officer for a local area hospice. Susan has been with Old Colony Hospice since 2017. Susan holds an associate degree in nursing (ASN) form Misericordia School of Nursing, Bronx, New York, Bachelors of Science (BS) in Legal Studies, Bachelors in Public Studies (BS) from Barry University, Miami, Florida, and a Masters in Health Science (MS) with a concentration in End-of-Life Care from George Washington University, Washington D.C.
Dr. McLain founded Old Colony Hospice in 1979. She is committed to ensuring that every patient is provided with the highest level of care specific to each patient’s unique needs. Dr. McLain brings a wealth of compassion and knowledge to the interdisciplinary care team at Old Colony Hospice and provides leadership and support to the team on a daily basis.
Dr. McLain received her Bachelor of Arts and her Medical Doctorate from the University of Missouri, Columbia. She is passionate about helping people and has served as a Volunteer Physician in Haiti since 2004.
