## Hospice Care
Hospice care is one of the most personal experiences of an individual’s life. Since 1979, Old Colony Hospice’s team of registered nurses, social workers, counselors, hospice aides, nutritional consultants and volunteers has been providing caring, compassionate hospice care to patients and their loved ones in partnership with the patient’s primary care physician. As a free-standing, non-profit, Medicare-certified, accredited healthcare organization, we are proud to serve over 60 cities and towns in Massachusetts in whatever setting they consider home.

## Palliative Care
Palliative Care is designed to provide emotional, spiritual and physical symptom management for patients and their families facing chronic, progressive or complex illnesses. The goal of palliative care is to improve quality of life through management of troubling symptoms.
