Old Colony Hospice is here for you 24 hours a day, 7 days a week, 365 days a year, to respond to your needs. We focus on offering patients and their families the physical, spiritual and psychosocial support they need in the environment that is most comfortable to them. Old Colony Hospice is committed to creating a patient and family experience that allows everyone involved to feel cared for, comfortable and supported during what can be a challenging time. We do this by offering support, comfort and choices to our patients and their families. In addition to expert medical care, we offer emotional and spiritual support that is personalized for each patient and family.

  • We are available to help you 24 hours a day, 7 days a week, 365 days a year.
  • Our nurses provide skilled nursing care for pain management and symptom control. They also provide education and support to the family. They coordinate patient care needs, arrange for services, supplies and medications, and coordinate care with the primary care physician.
  •  In addition to expert medical care, our social workers, bereavement counselors, and spiritual counselors offer emotional and spiritual support tailored to the specific needs of each family. We offer support groups and other community resources to help families adjust and understand the impact of the illness.
  • Our hospice aides help patients with personal care activities such as bathing, toileting, skin care, hair/shaving, dressing, light housework and meal preparation. All aides are certified home health aides. 
  • Our trained Compassionate Care Volunteers provide companionship, support and respite to patients and their families.  Visits may include specific tasks such as reading to patients, playing cards, helping with correspondence or running errands.  Sometimes, simply sitting and watching a movie, holding a hand, or being a caring presence in the room is all that is needed.
  • Provides visits from nurses, social workers, hospice aides, volunteers, spiritual care for end-of-life issues, and bereavement support to the family for the year following death.
  • Other specialized therapies and services include dietary counseling, physical, speech and occupational therapy.
