MAKING A REFERRALAnyone can make a referral to hospice—a
spouse, sibling, mother/father or other relative, a friend and, of
course, a physician or nurse. Referrals are made directly by phone to
800-370-1322. Old Colony Hospice will be glad to make a FREE
informational visit to you in your home, nursing home or hospital. Our
liaison nurse will also do a FREE assessment to ensure you are
appropriate for hospice care. If your physician has not made the
referral, we will contact him/her to discuss hospice care for you.
Ultimately, your physician will need to sign a form that states you are
appropriate for hospice care. |
