Social Worker

Old Colony Hospice has a current opportunity for a full-time caring and experienced social worker. The Hospice Social Worker, as a member of the Interdisciplinary Team, identifies psychosocial needs of the hospice patient/family/caregiver, establishes goals and interventions, and provides services in accordance with the hospice plan of care. Qualifications: must have MSW with a minimum of 2-3 years of post-graduate experience, strong medical social work skills and/or experience with death and dying; previous hospice is preference. Must also have excellent interpersonal skills, be computer literate and have reliable transportation.

Development Associate - Special Events

The Development Associate – Special Events will be responsible for the coordination and implementation of fundraising activities for Old Colony Hospice targeted towards the general public, major donors, memorial donors, and key constituents to increase donations, heighten awareness of Old Colony Hospice programs and services and make new friends and as identified through the agency’s strategic plan and the annual development plan. Qualifications include: 1-3 years successful experience in fundraising and development of special events in a nonprofit environment; Undergraduate degree or equivalent preferred; must have proficient computer skills with the ability to learn and apply specialized software; knowledge of Raiser’s Edge, a must

Hospice Aides

We are looking for part time certified and experienced CNA/HAs to join our busy hospice team. You will provide care for our patients in their homes or in skilled nursing facilities. All candidates must have: at least 1 or more years of prior hospice experience; excellent communication, organizational and interpersonal skills; professional appearance and demeanor; reliable transportation to travel between office, facility and patient homes; and also must have a demonstrated record of dependability.

Full time Hospice Nurses – Home Care and Skilled Nursing Facilities

We are looking for skilled and experienced hospice nurses who are knowledgeable and competent in end-stage disease symptom management and working as part of interdisciplinary team to provide case management in homes and also nurse to provide care in skilled nursing facilities for our patients, families and caregivers. The successful candidate must have valid and current RN license and be authorized to practice in the Commonwealth of MA; 3-5 years experience in a hospice, skilled nursing facility or medical/surgical nursing setting; excellent communication, interpersonal, organizational skills; proficiency using computer and learning and applying specialized software; must pass a background check and drug test prior to employment and have reliable transportation.
