Old Colony Hospice is hiring for a hospice nurse (either RN or LPN) at the Dr. Ruth McLain Hospice Home in Braintree.
Salary: From $32.00 per hour
Schedule: Weekend positions available from 7am - 7pm Saturday and Sundays (optional weekday, but not required). Inquire for more details.

Old Colony Hospice is seeking a Full Time Weekend RN. The Hospice RN visits patients as scheduled and as needed. This RN is responsible for identifying patient/family needs and for providing supportive care in accordance with the attending physician's orders and plan of care and Old Colony Hospice’s policies and procedures.
The weekend hospice RN will work closely with the other members of the Interdisciplinary Team and the patient’s primary physician in order to maximize the comfort and health of patients, families, and caregivers regardless of the care setting, to promote continuity and adherence to the hospice plan of care. The ideal candidate has hospice and/or home care experience and an abundance of compassion, emotional intelligence, and superior communication skills. This person must be flexible, a team player, and exercise excellent clinical judgment. The Hospice Nurse provides nursing care to patients in support of the RN Case Manager, the admission team, and others as needed. Proficiency with technology is required. Must document at the point of care and use technology to improve communication and patient care while protecting patient's rights to privacy and data security.
Schedule:
The weekend hospice RN will be scheduled for 36 hours per week but will be paid for 40 hours. The schedules for the available positions are:
Saturday, Sunday and one weekday or every other weekend plus 2 weekdays from 8:00 am - 8:00 pm or 9:00 am - 9:00 pm. There is some flexibility with the schedule.
Qualifications:
- Licensed to practice as an RN in the Commonwealth of MA (BS Preferred)
- Ability to use good judgment and discretion and to work effectively as a member or part of an interdisciplinary team
- Must have knowledge and understanding of applicable federal, state, and local hospice, Medicare, Medicaid, HMO regulations, as well as HIPAA and other regulations protecting personal health information of patients.
- Must be able to maintain reliable attendance and be flexible.

The Hospice Aide (CNA) is responsible for providing personal care and designated homemaking services for assigned patients and families whether delivering hospice care in the patient’s home or in a skilled nursing facility during a regularly scheduled shift, as well as provide weekend and holiday coverage as may be required or requested. Job description:
The Admissions Nurse is responsible for performing the initial admission, patient/caregiver needs assessments, and initiating the hospice plan of care and accompanying services between the time of admission and transition to a case manager nurse, as well as performing other general hospice nurse duties as may be required or assigned.
Essential functions include, but are not limited to:
· Perform initial admission patient/caregiver needs assessments including identifying physical, psychosocial, cultural, and spiritual implications that may influence patient/caregiver
· Determine patient’s appropriateness for hospice services, routine or hospice general inpatient, consistent with OCH admission criteria policies and in conjunction with the patient’s MD
· Initiate and manage the hospice plan of care and accompanying services between the time of admission and transition to primary nurse, including collaboration with the interdisciplinary team and patient’s primary MD
· Educate, review, evaluate and document the understanding and acceptance of the patient/caregiver in regards to all aspects of hospice standards and care to be expected
· Schedule assessment/information visits with potential hospice patients and their caregivers as requested by the Clinical Managers
· Provide specialized pain/symptom management related to end of life discomfort
· Provide specialized hospice training to family and/or to facility staff/contracted employees to ensure adequate care as may be required
· Share in providing on-call/continuous care coverage as may be needed, completing necessary documentation
· Completes timely informational meetings, assessments, and all paperwork/follow-up of patients referred; documentation must substantiate and justify hospice services and comply with state/federal regulations and accrediting body standards
Qualifications:
· Licensed to practice as a Registered Nurse in the Commonwealth of Massachusetts (BS preferred)
· Ability to use good judgment and discretion and to work effectively as a member or part of an interdisciplinary team
