Hospice Nurse - Hospice Home in Braintree - Weekends (Part-Time)

Old Colony Hospice is hiring for a hospice nurse (either RN or LPN) at the Dr. Ruth McLain Hospice Home in Braintree.

Salary: From $32.00 per hour

Schedule: Weekend positions available from 7am - 7pm Saturday and Sundays (optional weekday, but not required). Inquire for more details.

He/she will be compassionate and able to provide a patient-focused experience with a home-like feel for the residents of the Dr. Ruth McLain hospice house.

Duties of the nurse include:

Physical, emotional, and psychosocial care and under the direction of a registered nurse (RN)
Pain and symptom management under the direction of the RN
Collaboration with the team to provide what matters most to the patient and family at the end of life
Observe the basic health status and report changes or symptoms that need to be managed to the RN
Educate patients/caregivers regarding disease process and end-of-life needs
Share in providing on-call/continuous care coverage when necessary as supervised by the RN

Qualifications:

Graduate of an approved practical nursing program and licensed as an LPN in the Commonwealth of Massachusetts
Driver's License (Required)
Proficient with computer skills
1-year of nursing experience is preferred but not required.
Ability to use good judgment and discretion and to work effectively as a member or part of an interdisciplinary team
Must have knowledge and understanding of applicable federal, state, and local hospice, Medicare, Medicaid, HMO regulations, as well as HIPAA and other regulations protecting the personal health information of patients
Must be able to maintain reliable attendance and be flexible in managing time to respond to needs of the organization with availability for times that are outside of the established schedule for this position
Excellent organizational and communication skills
Must have proficient computer skills with the ability to learn and apply specialized software
Must have reliable transportation and current and valid driver’s license and able to travel from patient home, facility and to office as needed on a daily basis
Must be able to pass a pre-employment drug test and background check as well as able to provide 3 current professional references that can be checked and verified prior to employment

Weekend RN (Full-Time)

Old Colony Hospice is seeking a Full Time Weekend RN. The Hospice RN visits patients as scheduled and as needed. This RN is responsible for identifying patient/family needs and for providing supportive care in accordance with the attending physician's orders and plan of care and Old Colony Hospice’s policies and procedures.

The weekend hospice RN will work closely with the other members of the Interdisciplinary Team and the patient’s primary physician in order to maximize the comfort and health of patients, families, and caregivers regardless of the care setting, to promote continuity and adherence to the hospice plan of care. The ideal candidate has hospice and/or home care experience and an abundance of compassion, emotional intelligence, and superior communication skills. This person must be flexible, a team player, and exercise excellent clinical judgment. The Hospice Nurse provides nursing care to patients in support of the RN Case Manager, the admission team, and others as needed. Proficiency with technology is required. Must document at the point of care and use technology to improve communication and patient care while protecting patient's rights to privacy and data security.

Schedule:

The weekend hospice RN will be scheduled for 36 hours per week but will be paid for 40 hours. The schedules for the available positions are:

Saturday, Sunday and one weekday or every other weekend plus 2 weekdays from 8:00 am - 8:00 pm or 9:00 am - 9:00 pm. There is some flexibility with the schedule.

Qualifications:

Hospice Aide (Multiple Positions Available)

Old Colony Hospice and Palliative Care is hiring for full-time, part-time, and per-diem Home Hospice Aide & CNA positions.

Salary: From $20.00 per hour.

The Hospice Aide (CNA) is responsible for providing personal care and designated homemaking services for assigned patients and families whether delivering hospice care in the patient’s home or in a skilled nursing facility during a regularly scheduled shift, as well as provide weekend and holiday coverage as may be required or requested. Essential duties include, but are not limited to:

Provide direct patient care (whether in the home or other location) consistent with the written instructions and/or as assigned including: bathing, oral hygiene, nail/skin/hair and foot care, pressure relief, dressing and feeding, transferring positions or locations; report to the charge nurse upon arrival and before leaving a skilled nursing facility
Assist patient with prescribed exercises, with ambulation and repositioning as may be part of the plan of care or as directed
Provide light housekeeping duties such as: changing the bed, doing patient’s personal laundry, tidying the patient’s room, doing patient’s dishes, and keeping kitchen and bathroom neat and clean after use
Prepare and serve meals to the patient and/or prepare food to be served later
Maintain appropriate communication with the Hospice Aide Scheduling Supervisor and/or Clinical Manager on a daily basis including checking telephone messages, text or e-mail every morning before leaving to visit the first patient and between each patient visit.
Maintain accurate records/documentation of patient care given in the electronic record system application built into the organization-supplied smartphone

Admissions Weekend RN (Per-Diem)

Old Colony Hospice is looking for a per diem Admission RN for weekends.

Salary: Starting at $40.00

Job description:

The Admissions Nurse is responsible for performing the initial admission, patient/caregiver needs assessments, and initiating the hospice plan of care and accompanying services between the time of admission and transition to a case manager nurse, as well as performing other general hospice nurse duties as may be required or assigned.

Essential functions include, but are not limited to:

· Perform initial admission patient/caregiver needs assessments including identifying physical, psychosocial, cultural, and spiritual implications that may influence patient/caregiver

· Determine patient’s appropriateness for hospice services, routine or hospice general inpatient, consistent with OCH admission criteria policies and in conjunction with the patient’s MD

· Initiate and manage the hospice plan of care and accompanying services between the time of admission and transition to primary nurse, including collaboration with the interdisciplinary team and patient’s primary MD

· Educate, review, evaluate and document the understanding and acceptance of the patient/caregiver in regards to all aspects of hospice standards and care to be expected

· Schedule assessment/information visits with potential hospice patients and their caregivers as requested by the Clinical Managers

· Provide specialized pain/symptom management related to end of life discomfort

· Provide specialized hospice training to family and/or to facility staff/contracted employees to ensure adequate care as may be required

· Share in providing on-call/continuous care coverage as may be needed, completing necessary documentation

· Completes timely informational meetings, assessments, and all paperwork/follow-up of patients referred; documentation must substantiate and justify hospice services and comply with state/federal regulations and accrediting body standards

Qualifications:

· Licensed to practice as a Registered Nurse in the Commonwealth of Massachusetts (BS preferred)

· Ability to use good judgment and discretion and to work effectively as a member or part of an interdisciplinary team
