Our team of caregivers consists of nurse practitioners, registered nurses, home healthcare aides, social workers, spiritual and bereavement counselors and trained volunteers. Patients receive our services in their own homes or in nursing homes or assisted living facilities, as well as in hospitals or other institutions or facilities where the need may be. Our employees provide pain management, dignified comfort and compassion to patients and families facing the end-of-life. The patient and family make choices about their care and our team honors their wishes. Goals of care are met and quality of life is better because our specially trained staff focus on relieving symptoms so patients and loved ones can focus on more important things – like sharing love.
